| Incident Summary: | The goalkeeper from Central Valley was injured on a ball played into the box. In the investigation undertaken by the PAD Committee, it was found that at the time of this match Hollister did not have a Certified Referee Assignor and neither of the referees officiating the match were licensed at the time of the match. |
| Comments: | Hollister violated Rule 5.4 and 5.5 of the NorCal Handbook by failing to have a Certified Referee Assignor at the time of the game and failing to assign licensed referees to officiate the match. Hollister is fined $450 for failing to have 3 current year licensed referees officiating the match.
